NDIS Service Agreement

Thank you for your decision to order products and/or services from Mac & PC Doctors. Acknowledgement of this email constitutes a Service Agreement for products and/or services between Mac & PC Doctors and the NDIS Participant or his/her representative. This Agreement is made according to the rules and the goals of the National Disability Insurance Scheme (NDIS). The participant and MPD Accessibility agree that this Agreement is in line with the main ideas of the NDIS. These ideas include things like having more choices, achieving your goals and taking part in the community. By accepting this quote, you agree that the products you are committing to purchase are reasonable and necessary supports, related to the participant’s disability and in line with his/her goals. Mac & PC Doctors will provide the previously-mentioned products and/or services by accessing your NDIS funding via the NDIS portal. If you have indicated that your funds are plan managed or self managed, we will proceed to issue an invoice. You agree that Mac & PC Doctors will not be liable for any loss or damage that you or another person may suffer in relation to the purchase or use of these item/s under this agreement. Mac & PC Doctors agree to provide the goods and/or services agreed upon in as timely a manner as possible and for the agreed upon cost. We commit to treat your personal information with respect and confidentiality.
